 Your First Appointment:


Welcome to Children’s Heart Specialists! In order to facilitate your first visit to Children’s Heart Specialists, PSC, we would recommend that you read the information that appears below. Our main facility is located at 731 E Broadway in downtown Louisville. However, we have outreach clinics in Elizabethtown, Bardstown, Shelbyville, Danville and LaGrange. Please be sure to confirm in advance the specific location of your appointment. If you need directions please click on Office Locations.


A comprehensive cardiovascular evaluation may take 1 to 2 hours so please set aside 2 hours for your appointment. Make sure to bring your insurance card and co-pay amount or your deductible. We accept checks and most credit cards. Please call our billing office in advance (502) 584-3204 if you have a deductible or no insurance coverage. You will need to bring a referral to the office visit if you have Tri Care insurance. Other patients, like managed-care plan, may need to obtain authorization from your primary care physician.


We cannot evaluate a minor (under 18 years of age) without them being accompanied by a parent or designated adult. We strongly recommend that the legal guardian and caretaker of the patient come to the appointment as Dr. Villafañe will take time to explain the diagnosis and any other pertinent findings and recommendations. If for a justifiable reason the legal custodian is unable to come along to the appointment then he or she will need to fill out the following forms and sign a consent (click on CONSENT). You may like to print: REGISTRATION FORM, MEDICAL INFORMATION and HIPAA form, fill them out and bring them along with you.


Please be on time for your appointment or notify us 24-hours in advance if you cannot keep the appointment. We can use that time to see a sick child. Our staff is bilingual (English and Spanish). If you need a translator (and are unable to bring one with you) we must be notified at least 72 hours in advance by calling us at (502) 584-3200.
